Getting started with Lakeland Sports and Spine Physical Therapy is easy! Just follow these simple steps.

1. Make an Appointment
   
   Please call our office to schedule an appointment at 253-939-7179.
   
   
2. Gather Your Insurance Information
   
   Have your insurance information ready when you call us. Please call our office to verify if we take your insurance. Prior to your first evaluation and as a courtesy to you, we will call your insurance company to verify your benefits so we can let you know your specific benefits, deductible or co-payments. You are required to provide us with a valid insurance card and to pay for any co-payments and deductible co-insurance at that time. Also, make sure you call your insurance company to verify your coverage.
   
   

3. Doctor’s Referral
   
   You can see us without a physician’s referral. However, some insurance companies require a doctor's referral for reimbursement. If you do have your doctor’s referral, please bring it with you to your first appointment. The referral includes the name of your referring physician and his/her phone number.
   
   
4. Fill Out the Forms
   
   

   If you have already completed the above steps, and have a scheduled appointment, just download the forms below, print and fill them out, and bring them with you to your appointment! This will save you time when you come to see us. These forms include: • Patient History • Patient Consent • Patient Information • Office Policies
